Terms and Conditions Agreement

By registering and submitting payment on this website, I acknowledge that I have read, understand, and agree to the Terms and Conditions of the course I have selected, including the applicable Refund and Cancellation Policy. I understand that this constitutes a binding agreement between myself and Academy for Dental Assistants.

Payments & Authorization

I authorize the school to charge my selected payment method and acknowledge that I am responsible for all payments, including those made by a third party on my behalf (such as a parent, family member, employer, or sponsor). I confirm that all payments are authorized.

I understand that my payment secures a seat in a scheduled course and constitutes acceptance of all associated charges, services, and policies.

Payments are due as agreed at the time of registration, whether as a one-time payment or under a payment plan. For payment plans, payments are due on scheduled dates regardless of holidays, absences, class changes, withdrawal, or termination.

Enrollment constitutes a financial commitment, and all agreed payments remain due regardless of attendance or completion. Failure to remain current may result in restricted participation, removal from the program, and/or withholding of certificates until the balance is paid in full.

Returned or failed payments may result in additional fees and/or restriction of payment methods.

Registration & Forms

For the 12-Week Program, 6-Week Orthodontic Course, and 1-day courses, all required registration forms must be completed and submitted within three (3) business days of registering online in order to guarantee a seat in the class.

The school will hold a seat for up to three (3) business days; failure to submit required documentation within this timeframe may result in forfeiture of the reserved seat.

Forms may be submitted electronically as directed by the school.

Cancellations & Transfers (All Courses)

All cancellation or transfer requests must be submitted in writing via email to info@academy4da.com. The applicable Refund and Cancellation Policy for the selected course will be followed.

12-Week Program & 6-Week Orthodontic Course

Transfer Policy

Transfer requests must be submitted at least seven (7) days prior to the start date to avoid a $100 fee. Requests made within seven (7) days may be subject to a transfer fee.

 

Refund Policy

Refunds are governed by the signed Enrollment Agreement and are prorated only up to a certain point in the program, after which no refunds are issued.

  • Cancellation within three (3) business days of signing results in a full refund
  • Cancellation after three (3) business days, but before the first class, results in a refund minus the registration fee (not to exceed $150)
  • Withdrawal through 40% of the program results in a pro rata refund
  • Withdrawal after 40% completion results in no refund
  • Refunds are issued within 30 days of determination of withdrawal
  • If the school cancels a program, a full refund will be issued

All textbooks, workbooks, and materials are non-refundable and non-returnable once issued.

 

Program Policies

Failure to comply with attendance, academic, conduct, or payment policies may result in termination and may affect refund eligibility.

The school does not guarantee outcomes, including course completion, passing results, employment, or externship placement. The school is not responsible for securing or maintaining an externship site. Externship placement and continuation are not guaranteed and are subject to the discretion of the dental office.

 

Payment Plan Disclosure

By registering and selecting the deposit option, I acknowledge and agree that the total cost of tuition will be higher than the full tuition price due to a finance charge applied to payment plan options.

I acknowledge that:

  • The full tuition price is $2,795
  • The total cost when using a payment plan is $2,940, which includes a $145 finance charge
  • This finance charge is applied to payment plan options and distributed across the scheduled payments

I understand that by selecting the deposit/payment plan option, I am agreeing to complete the remaining balance through scheduled payments unless I choose to pay the remaining balance in full before the first day of class.

If the remaining balance is paid in full before the first day of class, the finance charge will be removed, and the total tuition will be adjusted to the full tuition price.

If I do not pay the remaining balance in full before the first day of class, I agree to the total payment plan amount, including the finance charge.

1-Day Courses (EFDA Certification, Dental Radiology Certification, Combo Certification, Refresher)

Cancellation & Transfer Policy
  • A minimum of seven (7) days’ notice prior to the hands-on (clinical) portion is required for refund eligibility
  • Requests made within seven (7) days are non-refundable
  • Transfers within seven (7) days are subject to a $100 transfer fee
  • Only one transfer may be granted without a fee; additional transfers may be subject to fees

 

Course Access & Completion

Accessing any online materials constitutes the start of the course and acknowledgment of services rendered, which may impact refund eligibility.

All courses must be completed within 90 days, after which access will expire with no refund.

Charge Disputes

By registering and submitting payment, I acknowledge and agree to all charges and policies associated with my enrollment. I agree to contact the school directly to resolve any billing concerns prior to initiating a dispute with my financial institution.

I understand that a charge dispute is not a valid method of requesting a refund. Failure to attend, complete the course, or achieve desired outcomes does not constitute valid grounds for a dispute.

Accessing, participating in, or receiving any portion of the course (online or in-person) constitutes acceptance of services rendered.

I further agree that disputes for charges related to any portion of the course accessed, attended, or made available are not considered valid, and that I remain financially responsible for all agreed charges.

Dental Radiology Certification & Licensure

After Dental Radiology certification, you’ll need to apply for licensure through the Florida Board of Dentistry. For questions about the application process, contact the board directly at (850) 245‑4474.

Academy for Dental Assistants is an approved education provider but is not responsible for the acceptance or denial of licensure and does not administer the licensure process. It is the student’s responsibility to ensure that all information submitted on their application is accurate and up to date. For any questions regarding licensure requirements or the application process, students must contact the Florida Board of Dentistry directly.

Fees

Any 3.5% credit/debit card processing fees are non-refundable.

Termination Policy

I agree to comply with all school policies and understand that Academy for Dental Assistants reserves the right to terminate my enrollment for violation of policies, including but not limited to attendance, academic progress, conduct, and payment requirements.

The school reserves the right to update or modify policies, and students will be notified of any changes.

Agreement

By proceeding with registration, I confirm that I have read and agree to these Terms and Conditions.

I acknowledge that I have not relied on any verbal or written statements outside of these Terms and Conditions and that this agreement represents the full understanding between myself and the school.

These Terms and Conditions, together with any signed enrollment or payment agreements, constitute the entire agreement between the student and the school.
